EMR
Find the process improvement solutions that best meet your needs today—and tomorrow. Today, FormFast solutions allow you to automate imprinting so all forms print on demand with a clear patient ID, reducing cost and improving ability to identify and read paper documents. When you are ready to go to the EMR, some portion of your process will involve scanning or COLD processing of existing forms and this is our specialty. We convert all forms to electronic templates which can be output with bar codes of patient and transaction ID for automated indexing of chart contents. The FormFast comprehensive partner approach incorporates assessment, recommendations, implementation and training, evaluation and follow-up. Our SmartLaunch package provides products and services to facilitate your transition to the electronic medical record.
Imaging
Common uses of our toolsets are the control of Imaging requisitions and reports to print when and how you want with bar codes and special fonts. Reports can be routed intelligently to appropriate physician based on workflow script, saved or forwarded.
Positive Patient ID
A Positive Patient ID that supports multiple patient identity systems with your choice of bar code symbology.
FormFast can generate a wristband and labels with bar codes that print out automatically in admissions, along with face sheets and other paperwork. If the wristband needs replacement or updating on the nursing division, FormFast offers an application program that looks at the ADT database and displays patients. The floor nurse can select the patient and item required —a wristband for example— and the laser printer of choice. All items will print on demand.
Barcoded wristbands offer an enhanced level of patient identification, security and safety because they:
- identify adults, mother/baby/significant other and pediatric patients in Admissions, the ER and on the floor
- are recommended by JCAHO
- are required by all current model glucometers
- are required to verify patient identity for phlebotomy
- offer a positive patient identification for blood transfusions
- support positive patient ID for medication administration
- provide patient ID for supply chain management
- offer a positive ID that reduces the risk of treating the wrong patient.
Electronic Signatures
Allow a patient to review consent and other signature-required forms and sign all the documents at once, using a specially designed pad. Patients can also review all signed documents on screen and receive a hard copy. Store an optical image of the consent forms with embedded signature in your facility’s database. This information can be retrieved as needed by other hospital departments, creating a totally paperless transaction.
Laser Checks Made Easy
Many solutions for laser check processing are expensive, complicated and cumbersome and do not provide a comfort level with respect to security. You are required to buy equipment, supplies and blank check stock at premium rates. You are not in control. Thus many clients are still seeking solutions. At FormFast, we keep it simple and secure. Because we have done hundreds of conversions using our own software, we have the experience to integrate this application to your system without fanfare in a couple of days. Expensive preprinted checks are replaced by generic check stock with a security background. The FormFast program allows reformatted data to be laser printed on blank check stock, stuffed in an envelope or processed as one page fold and seal self-mailer product

Admissions
Automate the Printing of your Documents for Patient AdmissionIn the typical system, admission forms are printed at one site and distributed to many areas. FormFast eliminates the preprinted forms and embossed cards, saving significant dollars. Admission personnel enter patient information in the same manner as they currently do and print using same "print" prompt as they use in their current system. There will be no separate screens to access. At the click of a print command, Admission packets with patient data are automatically distributed to the admission laser printer, business office laser printer, medical record, and nursing station laser printers at the same time.
With FormFast's Admission Solution, Admission personnel will not need to find and load pre-printed paper forms, tear down a form and distribute copies, or manually emboss or label forms. The solution can also recognize certain triggers and, based on the data, select and print Consent forms, ABNs, Privacy forms, Medicare forms or alternate language forms. Wristbands and labels with patient ID and bar codes can be automatically generated as part of the admission packet by using a dedicated bin on the laser printer or a thermal printer. As part of the print job, completed forms can also be automatically faxed or e-mailed to the admitting physician. For Same-Day-Surgery, ER or Nursing Floors, all clinical chart forms can be generated from admissions and printed in the appropriate location.
By eliminating the need for embossers and impact printers and by simplifying training, bedside registration is enabled. The FormFast registration module is also a platform for collecting patient signatures electronically and for scanning insurance cards for central access.
Scanning of Insurance Cards and IDs
FormFast has a scanning interface to allow registration personnel to scan cards or forms at the point of admission. These images can be accessed and viewed by business office or registration personnel. It is very common to use the FormFast front end to collect these images and deliver them to your optical imaging EMR database.
Eliminate Duplicate Key Entry of Pre-Admission Data
With FormFast, prospective patients or physician assistants can fill out pre-registration forms on-line. Patients go to the hospital website to a pre-registration section and fill out a web-based registration form. Data is then automatically entered to the hospital ADT system with prompts for Preadmission personnel to verify and edit as necessary. Preregistration is streamlined, time saved, errors reduced.
Business Office
Automating Business Office processes can quickly improve the financial performance of your hospital. With FormFast, standard Business Office processes are automated and handled immediately. For instance, when a patient registers at the Admissions desk, the coresponding Business Office forms and documents are automatically generated and printed in the Business Office.
Using FormFast solutions, business office personnel can leverage existing data from Registration, HL7 messages or HIS systems to automatically generate statements, invoices, UB04s, HCFA1500s or any other documents and print the standardized forms on plain paper with a laser printer. You control font size, color, consolidation of multipage documents, and the print location.
Also, using our access automation products, the business office has access to view insurance cards, face sheets and signed consent forms scanned or digitally collected in registration. Preadmission data is automatically entered to our database so that business office can review for insurance verification.
Human Resources & Administration
Administration will greatly benefit from the FormFastSite product which allows FormFastSite provides secure, web-based management of electronic and paper forms, policies, procedures and documents for client hospitals. This web repository is accessed online by all personnel or for patients/clinicians outside the fire wall as desired. See FormFastSite product under Solutions.
All administrative forms can be stored on FormFastSite with only administration controlled access to change, delete, update content so that administration, for the first time, controls sources and versions of forms.
All HR forms such as applications, employee evaluations, vacation requests can be completed and stored on the web.
Administration will also enjoy the benefit of report automation by which reports can be reformatted, consolidated and delivered via print production, secure email or fax as desired.

Eliminate Blue Cards
Your hospital's chart forms are digitized by FormFast graphical design software either by your personnel or by FormFast professional services and stored in a directory on the host system. The FormImprint program is accessible through any work station via web, Citrix or locally for down time purposes. It provides a menu which allows the nurse to select a user defined imprint and print it onto blank paper stock. The nurse can merge imprints for any patient or user-defined group of patients with any form or user defined group of forms (packet). For example, imprint data for a new patient could be merged with a packet called StartChart to print a completely new patient chart. Or, imprint data for all patients on the floor could be merged with a packet of all Daily Sheets to refresh documents at a shift change. Per the sample imprint on this form, imprint data can include data not normally shown, such as Height and Weight, and can be displayed in barcode representation if desired. There is a free text capability, which allows nursing to create templates for standing orders, teaching forms or any other purpose. FormImprint will also print wristbands or sheets of imprinted labels, with or without barcodes, if desired. FormImprint can be linked to your HIS system via HL7 or ODBC link when available or by use of FormFast Print Manager. The application is HIPAA and CCOW compliant.

IT/Information Systems
IT is the biggest winner using FormFast because the tools are easy to use and require little training for IS support personnel and no training at all for end users. IT can now implement electronic forms throughout the Facility without any dedicated FTEs in most cases.
